The Occupational Therapist will perform home health visits to evaluate patients' functional status and develop individualized intervention plans. They will also educate patients and their families on rehabilitative care and document all services provided using electronic medical records.
Candidates must hold a current Ohio Occupational Therapist license and have at least one year of recent work experience. Graduation from an accredited occupational therapy program is required, and prior home care experience is preferred.
