The Behavioral Health Home Visitor conducts weekly home visits and telehealth sessions to provide parent coaching, education, and case management. They also collaborate with the Child and Family Team to develop individualized treatment plans and connect families to community resources.
Candidates must have at least one year of experience in behavioral health, social work, or case management. Additionally, they must possess a valid Arizona driver's license, an insured vehicle, and an Arizona Fingerprint Clearance Card.
