Job Description: Who We Are: The Elliot VNA provides 24/7 in-home support and services to patients in Manchester and the surrounding communities.

Key Responsibilities

• Plans, develops, and implements physical therapy care plan appropriately.

• Regularly re-evaluates and updates appropriateness of care plan and physicians’ orders.

• Communicates appropriate information to patients, physicians, peers, managers, physicians, departments, and other agencies involved.

• Integrates problem solving methodology and quality improvement in resolving problems related to patient care and organizational processes.

Work Schedule

• Full-Time position.

• Weekend coverage may be required.
