About the Role

Join Covenant House New York as a Nurse Practitioner – Part Time, Primary Care in United States.

Key Responsibilities

(Responsibilities to include but not limited to the following) Elicits, records and interprets the past medical history, family and social history as it applies to the resident.

Functions as a member of an interdisciplinary team to plan, deliver and evaluate health care for a defined patient population.

Orders and performs necessary diagnostic tests, procedures and therapeutic interventions as identified in the plan of care.

These procedures are to be according to established protocols in which the PA is trained and approved to implement.

Requirements

and develop programs for a specific patient population, with regard to patient condition and age, and follow universal precautions.

Perform medical diagnostic, therapeutic procedures, and prescribe, dispense or deliver medications according to Covenant House protocol, collaborative practice agreement.

Completes timely documentation of patient assessment and plan of care in the electronic health record and generates appropriate health reports for outgoing correspondence.

Initiate and coordinate referrals to other health care team members or agencies.
