Key Responsibilities

• Provide direct patient care as a clinical pharmacist within scope of practice

• Collaborative care with medical, behavioral health, and care teams

• Deliver medication therapy management, chronic disease management, and patient education

• Clinical quality improvement and outcomes

Requirements

• Partner with external vendors (e. g., Nuvem) to ensure operational performance and regulatory compliance.

• Develop, implement, and maintain pharmacy policies, procedures, and standard operating workflows. Leadership, Governance & Reporting

• Serve as a key clinical pharmacy representative on applicable committees and interdisciplinary leadership groups.

• Provide pharmacy operational reports, metrics, and recommendations to senior leadership.
