About the Role

Job Title: Clinical Pharmacist Reports to: Chief Medical Officer Organization: The Healthcare Connection (THCC) Location: 1401 Steffan Ave. Cincinnati, OH About The HealthCare Connection: Founded in 1967, The HealthCare Connection was Ohio’s first Federally Qualified Health Center (FQHC).
Key Responsibilities
- Provide direct patient care as a clinical pharmacist within scope of practice
- Collaborative care with medical, behavioral health, and care teams
- Deliver medication therapy management, chronic disease management, and patient education
- Clinical quality improvement and outcomes
Requirements
- Partner with external vendors (e. g., Nuvem) to ensure operational performance and regulatory compliance.
- Develop, implement, and maintain pharmacy policies, procedures, and standard operating workflows. Leadership, Governance & Reporting
- Serve as a key clinical pharmacy representative on applicable committees and interdisciplinary leadership groups.
- Provide pharmacy operational reports, metrics, and recommendations to senior leadership.
