HCBS Waiver Case Manager Social Worker or Case Manager

Key Responsibilities

The case manager oversees waivered service delivery, develops and updates community support plans, and conducts assessments to determine client eligibility and needs. They also monitor provider performance, facilitate multidisciplinary team meetings, and provide crisis intervention and referral services.

Requirements

Candidates must possess a bachelor's degree in social work or an approved social services field and hold a valid driver's license. The role requires knowledge of social casework principles, family dynamics, and the ability to effectively manage a caseload while maintaining accurate documentation.
