Home Health PT (Physical Therapist)

Key Responsibilities

The Home Health PT provides treatment as ordered by physicians and assists in developing and updating patient plans of care. They are responsible for evaluating patient function, documenting clinical notes, and instructing family members or other personnel on therapy programs.

Requirements

Candidates must be graduates of an AMA-approved physical therapy school and hold a valid state license to practice. At least two years of experience is preferred, along with current CPR certification and a valid driver's license with an insured vehicle.
