Social Worker II (Central Access)

Key Responsibilities

• Respond to intake calls and assess for presenting issues.

• Conduct in-depth intake and targeted case management assessment.

• Develop targeted case management plan/goals in collaboration with client.

• Provide crisis intervention as needed and follow up.

Requirements

• Master’s degree in social work or related field; license eligible required.

• Current unrestricted CA Driver’s License and proof of auto insurance.

• Prior experience with client/ customer service engagement, and provision of case management required.

• Field-based service delivery and in-home visit experience preferred.

Benefits & Compensation

• Salary: $70K - $72K
