Key Responsibilities

Respond to intake calls and assess for presenting issues

depth intake and targeted case management assessment

Develop targeted case management plan/goals in collaboration with client

Provide crisis intervention as needed and follow up

Requirements

Master’s degree in social work or related field; license eligible required

Current unrestricted CA Driver’s License and proof of auto insurance

Prior experience with client/ customer service engagement, and provision of case management required

based service delivery and in

based programs and services. The Social Worker II will provide outreach to agency partners and community

based agencies. The position is both field and office

based and services will be provided both in person and through telehealth. Qualifications:

Master’s degree in social work or related field; license eligible required

Full-time position.
