What You Will Accomplish in This Role
- Performing history and physical examinations
- Documenting visits in electronic medical record in a timely manner
- Educating patients on their health conditions in a way they can understand.
- Following up with patients more frequently for effective condition management.
- Utilizing your support team to complete standing orders
- Ordering diagnostic tests based on assessments, developing treatment plans, prescribing drugs and other therapies as needed
- Making referrals as necessary, consulting with supervising medical provider when appropriate.
- Maintaining patient confidentiality
- Participating in meetings
- Working collaboratively with other members of the medical team
- Collaborating with the management team on quality improvement initiatives
- Having an open mind to feedback, different perspectives, reflection, and growth.
Who You Are and Your Experience
- Must have completed an accredited NP/PA program and be registered by the NYS Department of Education.
- 3 years or more experience practicing as a Nurse Practitioner or Physician assistant
- Experience working with a diverse and under-served patient population a plus
- Proven ability to work in a team environment eClinicalWorks experience is a plus

Pay: $140,000.00 - $180,000.00 per year (plus performance based bonus)

Requirements
Candidates must have completed an accredited Nurse Practitioner or Physician Assistant program and be registered with the NYS Department of Education. A minimum of three years of experience practicing as an NP or PA is required, with experience serving diverse and underserved populations being a plus.
