Job Description

Blue Circle Health is a nonprofit organization working to improve how type 1 diabetes (T1D) care is delivered and accessed. Funding from The Leona M. and Harry B. Helmsley Charitable Trust makes it possible to provide our 6-month virtual care, education, and support program to adults with T1D for free.

Designed based on need rather than traditional reimbursement rules, our whole-person model is individualized and can include endocrinology care, diabetes education, insurance navigation, peer support, diabetes supportive counseling, help paying for T1D medications and supplies, and connections to community resources.

We work alongside existing healthcare providers to fill gaps in care and empower people to manage this 24/7 condition.

We’re building a high-quality, scalable model for better T1D care—and we’re looking for mission-driven professionals who want to help create the healthcare system people with T1D deserve. Our ambition is to reach and support as many people living with T1D across the country as possible, and that urgency shapes how we work: we move quickly, set a high bar, and work hard to turn our mission into meaningful impact at scale.

Blue Circle Health is searching for an experienced MD or DO who will provide virtual patient care, specific to people with type 1 diabetes. Blue Circle Health has a team based, whole person approach to care where we bring together multiple specialties to create a personalized care plan for our patients.

The Endocrinologist will conduct telehealth visits and will work with the clinical care team to provide the best care possible to the people we serve.

The Endocrinologist will partner closely with the care team - including educators, diabetes supportive counselors, case managers, T1D support guides, and insurance navigators - to guide, educate and empower people living with type 1 diabetes to achieve better health outcomes.

Currently we offer 30 minute (follow-up) and 60 minute (new patient) appointments, as we believe spending time with each of our patients is critical. This team includes support from medical assistants and from our technology team.

This role is primarily remote, with potential to attend in-person meetings. All team members also attend our annual in person company retreat. Our patients are currently located in 21 states + Washington D.C., with the intention to expand to additional states throughout the country.

We will support the Endocrinologist in obtaining and maintaining licensure upon hire. This role is full-time and requires consistent availability Monday–Friday, with scheduled hours of 11:00 a. m.–8:00 p. m. ET on Monday, Tuesday, Wednesday, and Friday. Thursdays are scheduled either 8:00 a. m.–5:00 p. m.

ET (preferred) or 11:00 a. m.–8:00 p. m. ET. Blue Circle has a call schedule during the work-day but does not currently have off-hours call, however as we expand, this may be a future need. Candidates can live anywhere in the U.S.

  • An MD or DO who is a board certified/board eligible endocrinologist in the United States.
  • Must be eligible to obtain and maintain medical licensure in U.S. states where BCH provides care and have no known circumstances that would prevent or materially interfere with obtaining additional state licenses as BCH expands.
  • An approach to care delivery that embraces innovation and patient-centered care.
  • A strong communicator that can earn the respect of people with different backgrounds and expertise.

Preferred Skills and Experience

  • Experience providing care in a remote care delivery model.
  • Experience in working with under-resourced communities and diverse populations.
  • Experience using data analytics, population health metrics, and quality improvement processes.
  • Experience in oral speaking and presenting to a range of audiences.
  • Experience contributing to the design or oversight of Quality Improvement initiatives
  • Evaluate and treat our patients with T1D. Conduct medical evaluations, consultations, and provide diabetes-related care using telehealth platforms.
  • Provide guidance and care to our patients living with T1D, using evidence-based therapies and following ADA Standards of Care, with a focus on achieving individual goals and optimizing health outcomes.
  • In partnership with the Care Team, design individualized treatment plans for each patient, monitor their progress and adjust their therapies as needed.
  • Analyze data from all existing diabetes devices and stay up to date on future tech advances in the field
  • Teach patients, and new support staff, how to interpret device downloads and adjust insulin dosing and provide additional patient education to support this.
  • Address urgent patient inquiries, critical updates, and emergency interventions from patients and team members during work hours on a defined call schedule.
  • Provide medical guidance to a team of diabetes care providers including APRNs, RNs, diabetes educators, clinical social workers, behavioral health providers and potentially others as the team grows.
  • Direct supervision via state-specific collaborative care agreements with our current APRN with longstanding experience in the field, with potential to add additional mid-level providers in the future.
  • Partner with the VP of Clinical Care, VP of Clinical Operations, and team Endocrinologist(s) to provide support and guidance to the team responsible for the end-to-end patient care journey, ensuring that all services are delivered in a coordinated, efficient, and effective manner.
  • Help ensure that the clinical team provides compassionate and culturally responsive care and ensure rigor in evidence-based quality and safety.
  • Work with leadership and the administration team to ensure regulatory compliance of the clinical care team with local, state and federal regulations.
  • Use quantifiable evidence of impact to advocate for the improvement or scaling of services.
  • Develop partnerships with referring providers (PCPs, Endocrinologists, etc) and keep an open source of communication and camaraderie to support mutual patient care.
  • Potential to attend in-person meetings with partnering clinics to meet providers and present on our approach to care.
  • Lead by example in supporting a non-traditional healthcare model designed to serve both patients and providers to decrease barriers to care.

Compensation

Range: $220,000-$280,000/year. This range reflects the good-faith estimate of the compensation we reasonably expect to pay for this position. Individual compensation decisions are based on job-related factors, including skills, experience, and internal equity.

Requirements
