What You'll Do

* Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.

* Provide therapeutic interventions to improve strength, mobility, balance, coordination, and functional independence.

* Help patients safely achieve their mobility and functional goals in their home environment.

* Educate patients and caregivers on exercises, mobility strategies, fall prevention, and appropriate use of equipment.

* Collaborate with patients, families, physicians, and interdisciplinary team members to coordinate care and achieve optimal outcomes.

* Monitor patient progress and adjust treatment plans as needed.

* Complete timely and accurate clinical documentation in accordance with company and regulatory requirements.

* Maintain current clinical knowledge through continuing education and professional development.

What We're Looking For

* Current Massachusetts Physical Therapist license.

* Home health or outpatient experience preferred.

* Strong clinical assessment and treatment skills.

* Excellent communication and interpersonal skills.

* Ability to work independently and effectively manage a patient caseload.

* Strong organizational and time-management skills.

* Valid driver's license and reliable transportation.
