About the Job
We’re looking for a Family Nurse Practitioner to join our collaborative team in a part-time capacity working 20 hours per week. You’ll focus on early intervention, chronic disease management, and long-term wellness in a setting that values quality over volume.
Schedule: Tuesday: 11:00 AM - 7:00 PM
Wednesday: 8:00 AM - 12:00 PM
Thursday: 6:00 AM - 2:00 PM
What You’ll Do (Key Responsibilities in Patient Care)
- Comprehensive Care: Provide both in-person and virtual visits to increase access and continuity of care.
- Diagnosis & Planning: Prescribe medications and develop personalized health plans with your patients.
- Wellness Education: Educate patients on wellness, prevention, and lifestyle management.
- Collaboration: Collaborate closely with physicians, medical assistants, and other clinicians.
- Culture: Contribute to a positive, team-oriented clinic culture.
What You’ll Bring to the Team (Minimum Qualifications)
- Active and unrestricted FNP license in the practicing state.
- Board certification as an FNP.
- DEA registration.
- 2+ years of primary care experience preferred.
- Experience treating patients ages 2+.
- Familiarity with HIPAA and a strong commitment to patient confidentiality.
- Ability to pass on-site credentialing.
