St. Luke's Health-Memorial Livingston is a 66-bed facility and member of CommonSpirit Health, a national nonprofit health system.
Serving East Texas, St. Luke's Health-Memorial provides more than a quarter of a million patient services annually and contributes millions of dollars in local charity care and community support.
This facility delivers more than 60,000 patient services annually—half through its Level IV trauma-designated emergency department. Patients have access to a wide range of services including orthopedic care, women’s health and a state-of-the-art sleep center.
Advanced imaging services include MRI, CT, 3D mammography, 4D ultrasound, cardiac echo and more—all close to home.
Job Summary and Responsibilities
As our Physical Therapist, you will help patients rebuild or maintain their physical wellbeing so they can function at their best level, reduce pain, and improve their quality of life.
Every day you will perform initial evaluations, develop and update individualized plans of care, and provide a variety of direct treatments.
You will be expected to consistently meet patient care productivity standards, collaborate with the interdisciplinary team and families, and promote a positive customer service environment.
To be successful in this role, you must possess strong age-appropriate competency skills, excellent communication abilities, and a collaborative, patient-centered approach to care.
Develop and update appropriate plan of care in collaboration with the patient/family and related to the persons age and lifestyle with measurable long and short term goals and a defined D/C plan Understand the role of physical therapy in the context of the patients age, total needs perspective and environment
Perform age appropriate competency skills in area of practice and with respect to individual patient problems. Follow intervention protocols and alter plan of care appropriately. Discharge patient when goals have been met or medical necessity is no longer warrants intervention. Establish provisions and/or make recommendations
Consistently meet patient care productivity standard as established for job classification. Work cooperatively with supervisors and peers to contribute to the overall productivity of the team. Utilize support personnel as appropriate and available. Actively support and participate in patient coverage efforts across all divisions within the department as necessary
Promotes and Supports positive and professional customer service, while interacting with clients, guests and hospital personnel. Works collaboratively with all members of the interdisciplinary team, other departments and disciplines to assure patients and customer’s needs are met in a timely manner. Encourages the active participation of the patient and/or family in goal setting and patient/family training/education
Provides positive and constructive feedback to enhance departmental operations. Completes all necessary documentation as outlined by departmental policy and documentation review. This includes, but is not limited to, patient care charting, charge entry, completing peer chart and peer reviews, and documenting on daily schedule
Utilizes available information management systems to support departmental and hospital goals. This includes utilization of hospital wide e‐mail, information announcements, policies, annual computer mandatories, charge entry, and accessing patient care related information. Access and use of information will meet hospital and information management guidelines

Requirements
Candidates must hold a degree in Physical Therapy and be eligible for licensure in Texas. A Basic Life Support (BLS) or CPR certification is required within 14 days of hire.
