FeldCare Connects, an app-based referral network, is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Palmdale and the surrounding areas.

Key Responsibilities
The Physical Therapist will deliver quality home healthcare to patients of all ages by observing, recording, and reporting on their condition. They are responsible for determining the plan of care and completing documentation through an electronic medical record system.
Requirements
Candidates must hold a current, active, and unrestricted Physical Therapist license issued by the state Board of Physical Therapy. Completion of an accredited Physical Therapy program and reliable personal transportation are also required.
