The selected Advanced Practitioner Provider would serve as a link between inpatient and outpatient care as a member of the Baptist Health team. Location Baptist AgeWell Center for Health - San Marco 1325 San Marco Blvd, Suite 300, Jacksonville, FL 32207 Job

Responsibilities

Consults with patients about their current conditions, orders diagnostics, analyzes results, diagnoses conditions, and prescribes treatment strategies, including prescription medications. May also actively participate in research in the field and advocate for policies that support the needs of elderly patients.

Qualifications

Master's Degree - Nursing Certified Nurse Practitioner Licensed APRN 2+ years APRN experience desired Primary Location: Agewell Physicians Baptist Health’s care for the community spans generations.
