Impact Health is seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join our growing clinical team on a Per Diem (PRN) basis serving patients within skilled nursing care facilities in West Hartford, CT!!

Compensation and Benefits

Earning potential: 1099 (Fee For Service) Flexible Schedule: Monday – Friday. No on-call, weekends, or holidays.

Key Responsibilities

Assess, develop and communicate comprehensive treatment plans using evidence based best practices Documentation withing 24-48 hours along with placing relevant orders for diagnostics and medications Monitor and manage acute and chronic conditions Compliance with facility, state and federal regulations/protocols.

Support quality initiatives and process improvement plans. Collaborate with interdisciplinary team, nurse managers, physicians and specialists Educate and consul patients and families regarding course of treatment Qualifications

Qualifications: Active license as a Nurse Practitioner (NP) or Physician Assistant (PA) Minimum 2 years of post-certification experience Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred Proficient with EHRs and clinical technology Strong communicator with a collaborative mindset
