The Physical Therapist provides skilled therapy care and teaching to patients in their homes while acting as a case manager to coordinate comprehensive care plans. They are responsible for conducting assessments, ensuring regulatory compliance, and preparing patients for self-directed care within their community setting.
Candidates must hold a degree in Physical Therapy and a current license with the Minnesota Board of Physical Therapy. A current BLS certification is required, and at least 3 years of experience in physical therapy is preferred.
