Most case management jobs keep you at a desk. This one puts you in the room.

If you've looked at pediatric case management before, you know the version everyone is hiring for. A queue, a phone, a screen full of names you will never attach a face to, and a utilization target. You review care. You don't deliver any.

This isn't that.

Home Heart Florida is one of the fastest-growing pediatric home health companies in the state. We take care of medically complex kids in their own homes: vents, trachs, feeding tubes, children who would otherwise be living in a facility. Our Nurse Case Managers know their families by name, sit in their living rooms, and stay with them for years.

What Makes This Role Different

You own your caseload. Not a queue. You know every child on it, you know what the family is dealing with, and you are the person who makes sure the care plan matches the reality inside the house.

You'll have room to fix things. If an authorization process is broken or a handoff keeps failing, you tell us and we change it. We're small enough that a good idea becomes policy in a week, not a quarter. You'll report to our Global Administrator and work closely with the Director of Nursing.

You're getting in early. We're growing fast, and the case managers who join now will shape how this company runs clinically.

The Role

  • You'll manage a caseload of medically complex pediatric patients in Gainesville. Day to day, that means:
  • Building and updating care plans that reflect what is actually happening in the home
  • Owning prior authorizations and renewals so coverage never lapses on a family
  • Turning complicated hospital discharges into a plan the family and the field nurse can follow
  • Teaching parents and caregivers to manage medications, pumps, vents, and feeding tubes in plain language
  • Keeping physicians, payers, supply companies, and field nurses pointed at the same plan
  • Backing up your field nurses when a case gets hard
  • Keeping documentation clean, timely, and audit-ready before anyone asks for it
  • Who we're looking for
  • Active Florida RN license and current BLS
  • 2+ years of pediatric nursing, with meaningful time in high-acuity or complex care
  • 2+ years in home health
  • 2+ years of case management or care coordination
  • Working knowledge of Medicaid, managed care, and pediatric authorizations
  • Comfortable in an EMR and with basic spreadsheets and messaging tools
  • Valid driver's license and reliable transportation for local visits across Gainesville
