Key Responsibilities

The Social Worker provides essential social work services across various settings including acute care, maternal child care, emergency department, home health, and hospice for patients with acute, chronic, or terminal illnesses and their families. Key duties involve identifying psychosocial needs, developing care plans from a strengths perspective, and managing tasks like discharge planning, crisis intervention, and collaboration with the multidisciplinary team.

Requirements

Candidates must possess a minimum of a Bachelor's degree in Social Work, with preferred experience being one to two years in a medical field. Preferred licenses include Licensed Master Social Work or Licensed Specialist Clinical Social Worker, although a Licensed Bachelor of Social Work is the minimum requirement.
