Job Description

Presbyterian Homes & Services is seeking a Hospice Medical Director to join our team.

Optage Hospice is seeking a Hospice Medical Director to assume overall responsibility for the medical component of our hospice patient care program. Reporting to the Hospice Administrator, the Medical Director assists in the management and administration of Optage Hospice's clinical services and serves as our liaison to the local medical community, helping us fulfill our purpose of "Creating Smiles in the Eyes of Residents."

Responsibilities

The Hospice Medical Director provides medical oversight of the hospice program, certifies patient eligibility, and partners with the interdisciplinary team, attending physicians, and community providers to deliver compassionate, high-quality end-of-life care.

Key Responsibilities Included But Are Not Limited To

Provide oversight of the program's medical services to ensure an adequate and qualified medical staff for clinical service and coverage needs

Serve as the physician representative of the interdisciplinary group (IDG)

Review patient eligibility for hospice services; certify and recertify in writing that patients meet medical criteria for admission and subsequent election periods

Assist the interdisciplinary team in developing and implementing a plan of care coordinated with the attending physician

Render necessary medical management, in consultation with the attending physician, consistent with the plan of care and patient needs

Coordinate on-call physician services to provide 24/7 coverage, and support hospice staff in the absence of the attending physician or after hours

Perform home and inpatient visits for patient assessment and care, and perform or arrange palliative care consult visits as needed

  • Act as a resource to staff, patients, families, and attending physicians on pain and symptom management
  • Assist in developing and reviewing clinical protocols, emergency care procedures, and patient care policies
  • Participate in quality improvement initiatives and identify opportunities to improve patient care and safety

Maintain current knowledge of regulatory and reimbursement guidelines and integrate them into clinical practice

Serve as liaison to community physicians, providing consultation and education on hospice and palliative care admission criteria

Participate in continuing education for direct-care hospice staff and assist with orientation of new staff

Qualifications

Board Certified Physician Required

Experience in geriatrics, internal medicine, palliative medicine, or other relevant field of medicine required; work experience in a certified hospice program and administrative experience preferred

Physician licensure (MD or DO) in the States of Minnesota and Wisconsin required; board certification in Hospice and Palliative Medicine preferred

Demonstrated knowledge of hospice philosophy, medical aspects of hospice care, current research and trends, and pain and symptom management

  • Demonstrated leadership skills to provide appropriate direction and supervision to staff
  • Excellent time management and organizational skills; self-motivated, self-directed, and detail-oriented
  • Ability to work effectively with health care professionals, patients, families, co-workers, and the public, and to adapt to changing priorities
  • Valid driver's license and insured, reliable vehicle
  • Demonstrated compatibility with PHS's mission and operating philosophies

Pay Range

The estimated starting pay range for this role is $210,000- $260,000 per year. This good faith estimate is based upon PHS’s typical starting pay range for this role. Actual pay range depends upon many factors including each individual employee’s qualifications and market conditions and is subject to change.
