Occupational Therapist (OT) Home Health - Full Time

Key Responsibilities

Provide and oversee skilled intermittent care to patients in their homes, including the development and implementation of personalized care plans. Monitor patient conditions and collaborate with physicians and clinical staff to ensure high standards of care.

Requirements

Must hold a current California Occupational Therapy license and a valid CA Driver's License. Requires at least one year of experience in home health or hospice care and the ability to travel regularly.
