Key Responsibilities

• Designs and implements individualized treatment plans to address functional needs for each patient.

• Maintain open communication with the Rehabilitation Team regarding patient’s performance/progress.

• Initiates and maintains communication with interdisciplinary team regarding patient’s progress, needs, and discharge planning.

Requirements

• BLS CPR PHYSICAL DEMANDS: Must be able to lift, sit, stand, stoop and walk without restriction.

• WORK ENVIRONMENT: Work is typically performed in an adequately lighted and climate-controlled department or patient room.

Work Schedule

• Per Diem position.
