As a Physical Therapist at Custom Home Health, you will

* Deliver individualized care that empowers patients to achieve their health and independence goals.
* Collaborate with RNs and interdisciplinary teams to design and implement personalized care plans.
* Educate patients and their families on therapy techniques, exercises, and treatments to promote lasting well-being.
* Coordinate and administer treatments that enhance physical function and overall health.

WHAT WE’RE LOOKING FOR

* Licensed Physical Therapist: Must hold a valid Physical Therapy license.
* 1+ Years of Physical Therapy Experience: Passion for Excellence
* Home Health Experience a must
