Job description
Join Maple Heights Behavioral Health as a Social Worker PRN in Fort Wayne, Indiana, United States. This is a PRN position. PURPOSE STATEMENT: Plan, direct, define and coordinate individualized and group therapeutic support and services to patients with behavioral problems, psychiatric disorders, post-traumatic stress disorder, substance use and physical and emotional abuse.

Key Responsibilities
The Social Worker will conduct psychosocial assessments, develop individualized treatment plans, and provide crisis intervention for patients with behavioral and psychiatric needs. Requirements
Candidates must hold a Master's degree in Social Work and possess at least three years of related experience. State licensure as an LMSW, LCSW, or LSW is required, along with CPR and de-escalation certifications.
