Prava Therapy is seeking a full-time Physical Therapist (PT) to join our team within a premier, luxury resort-style senior living community offering Assisted Living and Memory Care in Frederick, Maryland.

This community emphasizes a holistic wellness philosophy—focusing on an active lifestyle, personalized care, life enrichment, resort-style amenities, and continuous engagement. Residents value independence, connection, health, and maintaining an active, fulfilling life.

Your Role

As a Physical Therapist, you'll lead clinical physical therapy care, performing evaluations and designing plans of care while embedding yourself as a trusted clinical partner within this vibrant community.

You'll

Perform comprehensive physical therapy evaluations and establish individualized, evidence-based plans of care across adult/geriatric physical therapy domains.

Deliver skilled treatment interventions addressing strength, balance, gait training, transfer mechanics, functional mobility, endurance, joint integrity, and active daily routines.

Target fall prevention, balance recovery, mobility device prescription and training, apartment safety, and environmental navigation across both Assisted Living and Memory Care settings.

Introduce, adapt, and train residents, caregivers, and staff on mobility equipment, assistive devices, compensatory movement strategies, and physical safety tools.

Leverage community features—such as fitness centers, life enrichment spaces, outdoor pathways, and therapy spaces—to practice real-world functional mobility and engagement.

Identify subtle changes in resident function, safety, or physical status to support proactive care and timely plan-of-care adjustments.

Educate residents, families, caregivers, and community staff on functional independence, fall risk reduction, safe transfers, and mobility support.

Collaborate with interdisciplinary team partners, physicians, community leadership, wellness directors, life enrichment teams, and caregivers.

Monitor resident progress, document evaluations and treatment sessions accurately and in a timely manner, and continuously adjust plans of care based on functional outcomes.

Participate in care coordination, interdisciplinary team discussions, and community wellness initiatives as appropriate.
