Key Responsibilities

• Provides case management support to clients and their families in person, over the phone, through use of text messaging and in the community.

• Understands MDFT model and uses concepts when appropriate to aid clients in understanding and managing their symptoms and their recovery.

• Collaborates with MDFT therapists, supervisor and other relevant clinical team members to coordinate and deliver services to clients. Works in close collaboration with assigned therapist or supervisor in the event that clinical or crisis intervention is needed.

• Maintains family and other collateral contacts to ensure quality, client-centered services.

Requirements

• Education: Bachelor’s degree in human services required.

• Experience: At least two (2) years’ experience in a substance use setting preferred. Demonstrated orientation toward serving people with substance use and/or mental health issues in their community required.

• Licensure/Certification/Registration: Valid driver’s license. Training: Will attend all required program trainings.

• Additional Qualifications: Bilingual (Spanish Speaking) required.
