The Nurse Practitioner will provide medical care and manage terminal illnesses for patients in a hospice setting, including conducting face-to-face eligibility visits. They will collaborate with an interdisciplinary team to develop and implement individualized plans of care to ensure patient comfort and symptom control.
Candidates must hold a master's degree and be a graduate of an accredited Nurse Practitioner program with current nursing licensure. A minimum of one year of experience as a Nurse Practitioner and one year of hospice or palliative care experience is required.
