Job Description

The VNA, one of several delivery networks for the Care New England Health System (CNE) is seeking a full time Hospice and Palliative Care Nurse Practitioner or Physician Assistant to join our collaborative team in Rhode Island. The VNA provides a broad spectrum of home health, hospice services for adults, and the terminally ill.

Position Highlights

Provide medical visits to patients with advanced illness in community homes, nursing homes and assisted living facilities

Conduct comprehensive History/Physicals and specific palliative care for malignant pain symptom management, advanced chronic disease symptom management, provide patient and caregiver education while supporting the patient’s provider care team

  • Daytime work schedule with autonomy with patient scheduling

Requirements

Graduate of an accredited Nurse Practitioner or Physician Assistant program.

Board Certified

Active Rhode Island APRN, RN, CSR, and DEA license or eligible.
