- Patient Evaluation & Care Planning Conducts comprehensive initial and ongoing assessments of patient mobility, strength, balance, coordination, pain, and functional status.
- Evaluates home environments for safety and recommends adaptive equipment or modifications as needed.
- Develops and implements individualized Plans of Care (POCs) based on patient needs and physician orders.
- Establishes measurable goals and expected outcomes to restore and/or improve function.
