Key Responsibilities

Conduct in-home health evaluations for health plan members to bridge gaps in care and provide comprehensive health assessments. Utilize clinical workflows to review medical history, perform physical evaluations, and facilitate diagnostic services while building trusted relationships with patients.

Requirements

Must hold an active, unrestricted physician license in the coverage area and be capable of obtaining multi-state licenses as required. Candidates must be comfortable performing clinical duties in diverse home environments and possess the ability to travel up to 100% of the time.
