Provides direct care to patients and serves as a consistent care coordinator through the continuum of care, assessing the physical, psychological, spiritual and social needs of the patient. Directs patients to the next appropriate service and tracks their progress.

In collaboration with Medical Director, assure that all hospital behavioral health patients receive appropriate evaluation, diagnosis, treatment, medical screening and medical/psychiatric evaluation whenever indicated, and that all medical/psychiatric care is appropriately documented in the medical record.
