Key Responsibilities

• Focus on whole-person care model for psychiatric clients.

• Provide direct services for intake, screening, and referrals to other service lines.

• Participate in the client’s ongoing assessments, diagnosis, and collateral treatment: serious mental illness, substance abuse, and co-occurring disorders.

• Coordinate post-hospitalization follow up care.
