Our Home Health Physical Therapists enjoy some excellent benefits: $90,000 - $105000/base salary Bonus opportunities EVERY pay period Mileage reimbursement for eligible travel between patient visits Build meaningful one-on-one relationships with your patients Flexible assignments, autonomy and work-life balance Clinician Friendly EMR Paid Time Off (PTO) and Holiday Pay Comprehensive Medical, Dental & Vision Insurance - employer pays 90% of employee-only medical plan 401(k) Retirement Plan to help you invest in your future Online training, growth and ability to earn CEUs Tuition discounts through Rasmussen University As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do: Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals Observe movements, conduct assessments, document progress and modify care plan as needed Assist patient with exercises and hands-on therapy to manage pain and increase mobility Educate patients on their plan of care, established goals, exercises and self-care Coach family on the plan of care, therapy goals and safely assisting with exercises A few must-haves for Home Health Physical Therapists: Graduate of an accredited Physical Therapist Program and active PT license in Minnesota Minimum of 1 year of physical therapy experience, ideally in home healthcare CPR certification Knowledge of state and federal home health regulations Good clinical judgement, strong interpersonal skills, dexterity and compassion Why Work for Interim HealthCare?

Key Responsibilities

Provide home-based physical therapy to patients to help them regain strength, mobility, and functionality. Collaborate with a multidisciplinary home health team to document progress and educate patients and families on care plans.

Requirements

Requires a graduate degree from an accredited Physical Therapist program and an active PT license. Candidates must have at least one year of physical therapy experience and current CPR certification.
