Key Responsibilities

• Provide biopsychosocial assessment and case management services as appropriate for children and youth who are ReDiscover clients.

• Assess children and families accurately and completely using all the diagnostic tools required.

• Develop treatment plans that match the needs of the child/family/community and works toward meeting goals of the treatment plan.

• Follows through with all tasks and interventions that are identified by all parties involved Integrates service provision to all parties to address the needs of the client.

Requirements

• of the Position Availability to work a flexible schedule to meet the needs of clients enrolled in the program.

• Master’s or Doctorate Degree in a social science field (as defined by the Department of Mental Health).

• The ability to work with all persons with multiple challenges (mental illness, substance abuse, physical challenges, developmental disabilities, etc.
