Licensed Physical Therapist (PT) – Home Health
HTS is currently seeking a dedicated and compassionate Physical Therapist to join our team in providing exceptional care to home health patients in Columbus, Indiana, and the surrounding areas. In this position, the Physical Therapist will provide therapy services to residents in their individual homes or onsite at our partner senior living facility. This is an excellent opportunity for a professional looking to make a meaningful impact on patients' lives while enjoying flexible work arrangements. Growth into a part-time or full-time position is possible for motivated individuals.
Job Details
- Assess patients' functional abilities and develop personalized treatment plans to improve independence and quality of life.
- Monitor and document patient progress, adjusting treatment plans as necessary.
- Educate patients and their families on therapeutic exercises and strategies to support ongoing health and wellness.
- Collaborate with healthcare teams, caregivers, and facility staff to ensure comprehensive patient care.
- Maintain accurate and timely documentation in compliance with regulatory standards.

Requirements
Applicants must have an active Indiana Physical Therapist license or must have the necessary qualifications to earn Indiana license. These qualifications include earning an advanced degree from a Commission on Accreditation in Physical Therapy Education-accredited, Physical Therapist education program, passing the state licensure exam, and fulfilling the state’s requirements for licensure.
Applicants must be legally eligible to work in the United States. Proof of eligibility will be required upon offer of employment.
