Key Responsibilities

will provide medical and social service case management, including helping with obtaining benefits, helping to access resources for housing, food security, transportation, refugee health needs, and other resources that may help the patient's health and wellbeing.

Requirements

BSW degree from a school of social work accredited by the Council on Social Work Education (CSWE).

3 years of social work experience preferred.

Must be CPR certified. Knowledge, Skills, and Abilities:

Strong written/verbal skills.
