The Occupational Therapist will conduct comprehensive evaluations in the patient's home to assess functional limitations related to ADLs and IADLs, identifying physical, cognitive, and environmental factors impacting independence. Responsibilities also include educating patients and families, monitoring progress, adjusting treatment plans, collaborating with the healthcare team, and maintaining accurate documentation.
Candidates must possess an active Occupational Therapy License in Arizona and hold a valid Driver's license with reliable transportation. Prior home health experience and residency in Phoenix or the East Valley are preferred qualifications.
