- vision, or Unit: UHS Primary Care Walton Primary Work Shift: Day Regular Scheduled Weekly Hours: 40 Compensation Range: $33.63 - $50.45 per hour, depending on experience ----- Education and Experience Minimum Required: Master’s degree in social work from an accredited program with licensure as Licensed Clinical Social Worker (LCSW) in New York State. Experience working with specific populations, such as individuals with mental health concerns, families, or older adults. Experience with electronic health records, ability to work independently and as part of a team and commitment to ethical and culturally sensitive practice. Preferred: Preferred 3 years of postgraduate experience. Key Responsibilities
Provide psychosocial evaluations, crisis and trauma counseling, and case management services for patients and families. Collaborate with an interdisciplinary team to monitor psychosocial status and facilitate discharge planning and decision-making.
Requirements
Requires a Master's degree in social work and a valid New York State LCSW license. Experience with mental health populations and electronic health records is required, with 3 years of postgraduate experience preferred.
