About the Role

Join Central Ohio Primary Care as a Registered Dietitian – Diabetes Management, M-F Schedule in United States.

Key Responsibilities

  • Provide medical nutrition therapy for COPC diabetes patients as referred from the primary care provider or Endocrinologist.
  • Offer individual counseling as well as group classes to provide nutrition education on diabetes to prevent complications with lifestyle related modifications.
  • Use motivational interviewing strategies to encourage positive behavior change. Work with the patient to create individualized goals and monitor progress through follow up visits.
  • Communicate regularly with the referring providers to discuss patient's progress. Coordinate care and appointments with additional health care providers.

Requirements

  • Document all progress notes and outreach attempts in the Electronic Health Record.
  • Manage a rotating schedule at several endocrinology and medical practices including the local travel between locations.
  • Contact and schedule nutrition referrals as needed
  • Serve as a preceptor to dietetic interns from accredited universities.
