Job Description

Position Summary

Diagnoses and treats individuals with speech, language, voice, and fluency needs. This role is grounded in building genuine, trust-based relationships with residents and their families - we believe every person, team member and resident alike, brings unique strengths and perspectives that make our community stronger.

Essential Duties

• Performs assessments and diagnoses, and develops treatment plans for language and swallowing disorders.
• Evaluates levels of swallowing, speech, and language difficulty and identifies appropriate treatment options.

Creates and carries out individualized treatment plans that address each resident's specific functional needs and personal goals.

Teaches residents techniques to improve sound production, voice, and fluency, tailored to what resonates with each individual.

Creates goals and objectives for residents to work toward, celebrating progress and working collaboratively through challenges.

Provides support and advice to residents, their caregivers, and family members with warmth and patience.

Counsels residents and families on ways to cope with swallowing and communication disorders, fostering trust throughout the process.

Builds strong relationships with residents over time, taking a genuine interest in their personalities, preferences, and everyday lives.

• Compiles thorough notes on residents’ progress and adjusts therapies and treatment plans where necessary.
• Documents resident care services according to approved medical and facility documentation procedures.

Collaborates closely with fellow team members, contributing to a supportive, inclusive workplace culture where diverse backgrounds and perspectives are welcomed and celebrated.

Qualifications

• Basic Qualifications & Experience
• Education: Graduate of an accredited therapy program.
• Experience: 2–3 years of related experience, or an equivalent combination of education and experience.
• Licenses/Certifications: Current Speech Language Pathologist state license; CPR certification.
• Excellent verbal & written communication, active listening, and social perceptiveness

• Working Conditions / Physical Requirements
• Physical stamina to lift, transfer, and assist residents through physical activities.

Requires frequent lifting and carrying of items weighing up to 50 lbs. unassisted, including assisting residents when needed.

Must be able to communicate clearly with residents in person and over the phone.

Visual acuity adequate to perform job duties, including visual examination of residents and reading information from printed sources and computer screens.
