Home Health Physical Therapist

Key Responsibilities

The Physical Therapist will plan and administer personalized physical therapy treatments in patients' homes to improve strength, mobility, and independence. They are also responsible for documenting patient progress, coordinating with the care team, and overseeing the performance of PTAs and Home Health Aides.

Requirements

Candidates must hold a current and unrestricted Physical Therapy license and a valid CPR certification. Experience collaborating with a team of healthcare professionals is required, and one year of experience is preferred.
