Part-Time | PRN Brookestone Home Health Care

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our team on a Part-Time or PRN basis. This role offers the flexibility and autonomy to provide individualized care while building meaningful relationships with patients and their families throughout the Omaha area and surrounding communities.

Position Overview

As a Physical Therapist, you will evaluate, develop, and implement personalized treatment plans designed to improve strength, balance, mobility, and overall function. You will collaborate with an interdisciplinary care team to deliver exceptional patient-centered care in the home setting.

What You'll Do

  • Perform comprehensive physical therapy evaluations and reassessments.
  • Design and implement individualized treatment programs.
  • Provide therapeutic exercises, gait and balance training, transfer training, and fall prevention interventions.
  • Recommend and assess the need for assistive devices and durable medical equipment.
  • Educate patients and caregivers on treatment strategies and home exercise programs.
  • Track progress and adjust treatment plans to achieve optimal outcomes.
  • Collaborate with physicians, nurses, and other healthcare professionals to coordinate care.
  • Complete accurate and timely clinical documentation.
  • Adhere to agency policies, professional standards, and regulatory requirements.

Qualifications

Required

  • Current Nebraska Physical Therapist license in good standing.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and proof of automobile insurance.
  • Ability to travel within the service area.

Preferred

  • Previous home health experience.
  • Experience working with geriatric and rehabilitation populations.
  • Knowledge of Medicare regulations and home health documentation requirements.
