Key Responsibilities

• manage the delivery of support services to residents including the identification and assessment of psychos.

• lead abuse prevention programs as an integral part of an interdisciplinary team and are also responsible.

• Provide a smooth transition from an acute care environment, with special consideration on developing a plan.

Requirements

• Bachelor's degree from a school of social work accredited by the Council on Social Work Education

• Master's or Doctoral degree from a school of social work accredited by the Council on Social Work Education preferred

• Hawaii State Social Work license or eligible.

• BLS Certification.
