Join Lightways Hospice and Serious Illness Care as a Pediatric Social Worker – Hospice, Field Position in Joliet, Illinois. Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.
Key Responsibilities
- Assesses each patient/family’s unique social and emotional needs as indicated.
- Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required.
- Carries out social evaluations and plans intervention based on evaluation findings.
- Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties.
Requirements
- of one year's experience in health care, hospice experience preferred.
