Job Description
Position Summary:
Our Social Workers maintain a caseload of Community LIFE participants (adults over age 55) and provide them and their families with psycho‑social assessments and interventions related to the maintenance of the participants in the community.
As the primary contact, the Social Worker performs utilization case management for all acute inpatient admissions and transfers. This position is Monday-Friday 7:30am-4:00pm with no nights or weekends! We have a part time (3 day per week) opportunity available also.
Location: Homestead, PA
Required Education
Master’s Degree in Social Work (MSW) from an accredited school of social work.
Required Experience
One year experience working with the frail or elderly population. Geriatric social work experience preferred in a hospital, long term care or community setting.
Required Skills
- Good verbal and written communication skills.
- Ability to work with people from various socio‑cultural backgrounds in an effective manner.
- Ability to accept and utilize supervision.
- Ability to work independently and be self directed.
- Good problem solving skills.
- Must have a compassionate and empathetic attitude for the frail older adults.
Required Certifications/Licensing: Valid PA Driver’s license

Requirements
Candidates must possess a Master’s Degree in Social Work (MSW) from an accredited school and have one year of experience working with the frail or elderly population. Preferred experience includes geriatric social work in a hospital, long-term care, or community setting, along with a valid PA Driver’s license.
