Social Worker (MSW)

Key Responsibilities

Social Workers maintain a caseload of Community LIFE participants (adults over age 55), providing psycho-social assessments and interventions to support their community living. The Social Worker acts as the primary contact, performing utilization case management for all acute inpatient admissions and transfers.

Requirements

Candidates must possess a Master’s Degree in Social Work (MSW) from an accredited school and have one year of experience working with the frail or elderly population. Preferred experience includes geriatric social work in a hospital, long-term care, or community setting, along with a valid PA Driver’s license.
