Key Responsibilities
The physician will provide outpatient primary care to a senior patient population with complex chronic conditions within a value-based care model. Responsibilities include managing small patient panels, collaborating with a dedicated care team, and ensuring continuity of care to improve long-term patient outcomes.
Requirements
Candidates must hold an MD or DO degree and be board certified or eligible in Family or Internal Medicine with an active, unrestricted medical license. A minimum of 2 years of outpatient experience and proficiency in basic office procedures such as suturing and biopsies are required.
