Key Responsibilities

• Using the Practice Management system to reschedule Annual Wellness Visits as needed and in accordance with provider specific preferences.

• Must have clear communication with your patients, team members, and manager.

• Other Duties/Responsibilities: Attend all meetings requested by the management team.

Requirements

• Coordinates and schedules appointments for procedures at the clinic, ambulatory surgery center or hospital.

• The AWV Navigator II will be the designated trainer for new team members and provide training and support for others in the team as needed at the direction of the Population Health Manager.

Work Schedule

• Full-time position.
