Behavioral Health Home (BHH) Wellness Coordinator - Children Services

Key Responsibilities

The Wellness Coordinator will assist clients in managing and coordinating medical and mental health services while identifying necessary resources to support wellness objectives. Essential duties include maintaining knowledge of Behavioral Health Home requirements, providing integrated physical and mental health care, completing assessments, developing service plans, and managing a caseload.

Requirements

Candidates must possess a Bachelor's degree in social work, Psychology, Social Services, or a related field, or have one year of equivalent social service experience if the degree is in an unrelated field. Required abilities include working both independently and as part of a collaborative team.
