Key Responsibilities
Provide telephonic and telehealth clinical care to patients, caregivers, and facility staff across various settings. Coordinate care transitions and manage both acute and chronic medical and behavioral conditions in collaboration with the interdisciplinary team.
Requirements
Requires an active, unrestricted Nurse Practitioner license in New York and a Master of Science in Nursing or Doctor of Nursing Practice degree. Candidates must have at least 3 years of clinical experience as an Advanced Practice Clinician and hold a national board certification.
