About the Role

Join Trinity Hospice as a Full Time Hospice Social Worker in United States. Our chosen candidate will support patients in the Conroe, Livingston and surrounding areas. At least one year of experience as a Social Worker working for a hospice agency/hospice program is required to be considered, as well as successful completion of a Masters' Degree program in Social Work.

Key Responsibilities

• will support patients in the Conroe, Livingston and surrounding areas.

• will provide social services under the direction of a Physician, to offer support to our patients and their families.

• will be based on the patient’s psychosocial assessment, and the patient and family’s needs.

• Time Minimum Experience Experienced

Work Schedule

• Full-Time position.

• Hybrid position - combination of remote and on-site.

• Call responsibilities required.
