Join Trinity Hospice as a Hospice Social Worker – MSW, On-Call Rotation in Houston, Texas. Our chosen candidate will support patients in the Conroe, Livingston and surrounding areas. At least one year of experience as a Social Worker working for a hospice agency/hospice program is required to be considered, as well as successful completion of a Masters' Degree program in Social Work.

Key Responsibilities
The Social Worker will provide essential social services under physician direction to support patients and their families based on psychosocial assessments and identified needs. This role involves participating in the coordination of care with the Interdisciplinary Group and adhering to agency on-call rotations as required.
Requirements
Candidates must possess a Master's Degree in Social Work from an accredited program and have a minimum of one year of experience working as a Social Worker specifically within a hospice agency or program. A current driver’s license, reliable transportation, and valid auto liability insurance are also mandatory requirements.
